Geriatric Care Management can be assigned to nurses and medical social workers who are tasked to perform assessment, coordination, and advocacy across patient-to-patient cases.
Basically, their assistance permits the continuum of care for a senior by initiating an assessment of his/her ability to live in independence, use the data result for creating a care plan along with a care staff, and may provide on-going monitoring and intervention upon the care plan’s visibility in action.
Moreover, Geriatric Care Managers are responsible for:
- Assisting in coordination of services
- Interact proximately with the patient and the patient’s family regarding the level of needed services
- Develop a care plan suiting the need of the patient
- Provide knowledge in various community alternatives and resources
